The person signing needs a government-issued photo ID that is current, or was issued within the past five years: a California driver's license or ID card, a U.S. or foreign passport, or a military ID. When someone comes in through the trauma center or ER the ID is often still at home. Have someone bring it in if they can: California only allows the credible witness route when the patient has no ID to present at all.
The patient also has to be awake, aware of what they're signing, and able to tell us in their own words that they want to sign. We can't notarize for someone who is sedated or unresponsive.
