What the Patient Will Need
The person signing needs a current government-issued photo ID — a California driver's license or ID card, a U.S. or foreign passport, or a military ID. If their ID is at home or was misplaced during admission, that's not necessarily the end of the road: California law lets us confirm identity through one or two credible witnesses who personally know the patient and have their own valid ID. The patient also needs to be alert, aware of what the document says, and able to tell us themselves that they want to sign it.
